Periorbital hyperpigmentation is a common indication of chronological aging and photoaging. The contrast of the dark areas in relation to the surrounding skin draws attention away from other features and produces a general look of fatigue. Blepharoplasty is recommended in many cases of periorbital hyperpigmentation.

Factors that Lend to Periorbital Hyperpigmentation

Excessive pigmentation in the lower periorbital area is typically linked to genetic predisposition. The skin contains higher levels of melanin compared to the general population. As the aging process diminishes collagen levels and thins the skin, hyperpigmentation is more visible. This accounts for the chronological aging process that contributes to dark circles under the eyes. The effect is compounded when environmental factors are taken into consideration, such as poor diet, dehydration, poor sleep habits, and smoking. Inflammation of the tissues, skin laxity, eye rubbing, stretching the skin, and allergies are also known causes.

Correcting Periorbital Hyperpigmentation with Blepharoplasty

Lower blepharoplasty is a surgical intervention that removes excess skin and fat from the periorbital area. The surgeon may also opt to use fat grafts to restore volume in the skin, making it thicker to mask the hyperpigmentation. Often, the appearance of dark circles is linked to soft shadows due to the reduced tissue volume, and blepharoplasty may also remedy this issue. Lower blepharoplasty is a relatively straightforward procedure. An incision is made along the rim of the eyelashes to provide access to the internal tissues. Fat grafts may be inserted through the incision prior to the tightening of skin and excision of any excess. The procedure may also involve removing small amounts of fat to produce a smooth appearance on the lower eyelid area.

Blepharoplasty Aftercare and Recovery

Recovery varies somewhat by the patient, but the average recovery is seven to 10 days after blepharoplasty for dark circles. Patients rest at home with the head elevated to minimize swelling, using cold compresses to manage discomfort. The eyes are not used for tasks that require long periods of focusing, such as reading or watching television. Pain medications may be required for comfort, and the patient is given eye drops to help with dry eye symptoms.
